To find this information you need to go to your manager and click on 'Live'. Here you will find all the information you need to set up Nicecast.
Once you have correctly filled in the form you can close the window - you've set up your server on Nicecast.
Nicecast doesn't need a user name, just a password that must be in the form username:password.
